ACA Compliance Management System

Tracking, Reporting and Filing

Our ACA compliance tool provides employers with a thorough recordkeeping and reporting solution at an
affordable cost. The automated solution tracks, determines and documents eligibility status for all employees
using client data from payroll or time-and-attendance plotforms. The system also supports all associated
filing required by the IRS.

Hours and Eligibility Tracking

The ACA provides employers with complex rules regarding measuring hours and verifying full-time status. Our
module helps employers manage this process by tracking hours, determining eligibility, managing overlapping
timelines and documenting plon eligibility during the appropriate time periods.

Once customization is complete for the client, the platform provides reports on the eligibility determinations,
as well as dashboards of individual employee timelines. Our solution keeps an archive of employee
information for on-demand auditable recordkeeping.

Reporting and Filing Requirements

The ACA created new reporting requirements under Internal Revenue Code Sections 6055 and 6056. Under
these new reporting rules, certain employers must provide information to the IRS about the health plan
coverage they offer (or do not offer) to their employees.

Marketplace Notices

The ACA also created o requirement that employers distribute a Notice of Coverage Options to all employees,
regardless of plan eligibility, within 14 days of their date of hire. Our solution can generate and distribute
these notices within the allotted timeframe to ensure employer complionce. The notices can be customized all
the way down to the individual specific information on page 3 and include a custom cover letter with
employer messaging.

variances, which shall be investigated and reported, and corrective actions taken, as well as
preparation of projections of annual expenditures in order to anticipate areas of shortfall or
surpluses expected at year end.

. Quarterly Activity Report.

Contractor shall provide to the CTAB quarterly reports during regularly scheduled monthly public
meetings. Each quarterly report must include, at a minimum:
1. the total number of programming hours made available to the public on each channel and
through the use of other technologies such as webcasting;
2. asummary of the programming sources and hours for each channel, including:
a. new programming produced using City facilities or equipment, including original
and repeated playback;
b. events, programs and any other form of community announcements including bulletin
board slides;
3. areport of all complaints received by the Contractor during the reporting period, including
copies of any written complaints; and
4. any other information that either the City or the Contractor believes relevant to show
achievement of stated performance goals.

. Annual Report
Contractor shall file no later than the sixtieth (60th) day after the close of the fiscal year an annual
report containing, at a minimum:

1. the Contractor’s organizational chart, listing LLC Members and staff by function;

the number of people employed by Contractor, both full and part-time/number of independent

contractors;

an annual inventory summary;

a summary of quarterly programming source & hours;

5. aschedule of all fees and charges levied by Contractor to the City, with the percentage change

from the previous annual report (and all fees and charges levied upon producers & users if

any);

a summary of all programming outages experienced, noting root causes and remedial actions;

7. acopy of the complaint and appeal procedures used by the Contractor, any changes made, a

general description of the types of complaints and appeals received and the dispositions

thereof;

a copy of the Operations Development plan;

9. the number and types of persons using the Facilities, such as organizational or individual,
represented as totals and as percentages of the overall number of users, categorized by place
of residency (and such other categories as were used in the annual viewer survey, and in the
contract initiation survey);

10. acopy of equipment maintenance logs;

11. a list of training classes, schedules, and rates (if any);

12. the annual fundraising report, detailing all funding sources and how the funds were allocated;

13. annual survey of producers & viewing community(see Section 3.C.3.b), which shall include
but not be limited to demographic information and customer satisfaction; survey instrument

Assumptions

90-Day Disclaimer

The information (data) contained in this proposal constitutes a trade secret and/or information that is
commercial or financial and confidential or privileged. It is furnished to City of Nashua in confidence with the
understanding that it will not, without permission of Alight, be used or disclosed other than for evaluation
purposes; provided, however, that in the event a contract (or other agreement) is awarded on the basis of this
proposal the prospect shall have the right to use and disclose this information (data) to the extent provided in
the contract (or other agreement). This restriction does not limit the prospect’s right to use or disclose this
information (data) if obtained from another source without restriction.

This quote is valid for 90 days from the date of this proposal.

Reporting

Client is responsible for delivering timely and accurate data in Alight’s required data file formats. If Client fails
to deliver the data in the correct format, and if after notice and opportunity for Client to correct, Alight must
manually correct the data format, then Client will incur a fee of $150.00 per hour for all manual data
manipulation or correction.

During implementation, to ensure accurate IRS reporting of employee status and measurement timetines,
Client will be required to provide Alight with historical employment history ond hours worked, measurement
periods, benefit enrollment, and special leave of absence information. Client will provide such data to Alight
using Alight’s ACA Compliance data file format. To process such information, Alight wilt charge a one-time fee
equal to $0.50 per employee per month for each month of historical data imported, such fee not to exceed a
total of twelve (12) months, as provided in the Statement of Services. Some Clients may be asked to provide
more than 12 months of data to ensure accuracy (but the fee will not exceed a maximum of 12 months).

Agreement assumes Client will provide historical data in one file (rether than multiple date-range files).
If Client adds one or more affiliated entities to the platform after Client's initial implementation (for example,
through acquisition or merger), the historical data fee may apply to each such entity.

CHANGE ORDERS

Client will assume responsibility for costs associated with changes ta items and/or services not included in the
proposal. Change Orders should be sent to Alight and are subject to approval by Alight, based on timeline
requirement.
