Safety Equipment Purchase

To approve the purchase of the Gorbel Road Ranger Fall Protection Unit from
Mass Crane and Hoist Services, Inc. of Tyngsboro, MA in the amount not to
exceed $46,202. Funding will be through Department: 169 Wastewater; Fund:
Wastewater; Activity: Davit ARM Replacement.

The Wastewater Department has unique safety needs, with many tanks being
greater than 20 feet in depth and requiring personnel entry in order to clean and
maintain the structures. Recently we began a Facility wide project to increase
safety in the way we enter tanks and pump stations, with the installation of Davit
arms in the Wet Weather Facility Screening Upgrade, the Primary Upgrade, and
the Pump Station Upgrade.

In order to easily and cost effectively address the safety in the majority of areas,
the wastewater department would like to purchase a mobile fall protection unit.
The Facility explored two units, each that would meet the needs of the Facility.
One unit is the Gorbel Road Ranger, the other being the Malta Dynamics X1250.
We could not find a third comparable unit. The Gorbel Road Ranger has a lower
cost and meets the application needs of the Facility, making this the
recommended purchase. The quote received for the Gorbel Road Ranger was
from Mass Crane and Hoist for $46,202, which includes delivery.

Activities:

Waldron arrived on site at 0830 and met with Methuen at the Methuen trailer then proceeded to the Energy
Recovery Building to review items, NWTF met at the Energy Recovery Building.

The field review was to investigate electrical conduit discover in the slab and the internal CMU walls following
the start of the work done for the revised engine pad design captured in ECN-002. The following is a summary
of the conclusions of the field review. See Figure 1 for a snip from drawing S-1401 keying each of the impacted
locations:

Two holes were cut in the existing slab at this location and existing conduit was discovered, see Picture |
below. One of the two hole will be used to create a new engine foundation and the other will be recast with
concrete, One of the holes was cut in the location shown in ECN-002 drawing S-1401 (left) and the other (right)
was cut in the slab to the east at the request of NWTF in an effort to move the foundation and engine frame out
of the access aisle and reduce the potential of.a tripping hazard.

In the case of either hole being used for the foundation, the hole that will be used for the foundation will require
all conduit to be removed and rerouted as required and the hole not used for the foundation will require all
existing conduit be repaired and/or be removed and relocated prior to being recast with concrete.

In the case of the “right” hole to be used for the engine foundation, all conduit must be removed as discussed
above as well as there is expected to be a ductile iron drain line that will require protection. To protect the drain
line from forces transmitted through new engine foundation, the hole should be excavated down past the
minimum required depth as stated on S-1201 to expose the drain line. See Detail | below showing how to
protect the drain line. After the detail is constructed around drain, the hole can be backfilled and compacted to
the minimum depth shown on S-1201 and the remaining can be cast as shown in the original detail on 5-1201 or
the drain with protection and the entire hole can be reinforced and cast.

In addition, the “right” hole can be extended south approximately 4” to avoid additional conduit in NE wall
penetration. See further detail below.

WE Location:

The hole was cut in the existing CMU wall as shown on S-1401 and existing conduit was discovered, see
Picture 2 below, The conduit in the future location of the steel shall be removed and relocated.

It was determined that if the steel for the engine foundation was shifted to the south side of the penetration, as
shown in Picture 2, One (1) existing steel conduit can be avoided, see highlighted. Any other conduit should be
repaired if damaged and then can be cast in the wall with the new steel shape in accordance to the detail on S-
1401. This would require the “right” foundation at the NW location, see above, to extend to the south
approximately 4 inches to allow for the steel shape centerline to land on the centerline of the new foundation.

SW Location:

The hole was cut in the existing slab in the location as shown on S-1201, see Picture 3 below. Conduit was
found in the slab. All conduit remaining or that was damaged during removal of slab shall be removed and
rerouted as required.

SE Location:

The hole was cut in existing CMU interior wall as shown on S-1401, See picture below. Based on the centerline
of the SW foundation, the new steel in this wall penetration should avoid existing conduit. Any conduit that was
damaged shall be repaired and/or removed and relocated. Remaining conduit and new steel for engine
foundation shall be cast into wall as shown on S-1401.

Hi John,

As we discussed the Wastewater Department is requesting emergency approval of a Change Order for the Water Booster Statin project. Below is detail
information from Bill Keating, Wastewater Engineer on the change order needed and why emergency approval is required. Though this Change Order is
under $25,000, a previous Change Order #1 in the amount of $5,961.83 has already been processed bringing total Change Orders over $25,000.

Below is a summary of the correspondences with the Contractor regarding the proposed change order along with backup costs for the Water Booster Station
project:

9/12 -The Contractor’s subcontractor doing he earthwork, NE Earth Mechanics, was digging on west side of water booster station to determine location to
install new water valve that will connect to new water pipe and water booster station. After digging and exposing existing water pipe, contractor determined
they could not install taping machine with fitting at that location.

9/19 — contractor resumed work to locate a connection point to existing water pipe. They located an electrical duct bank and 2 other water lines crossing the one
they need to tap into. The water lines and duct bank were not shown on the design drawing due to lack of record drawings showing these utilities. There was no
way of knowing what buried utilizes existed without accurate record drawings. The contractor determine a tap location where there is 9-feet of pipe {clearance
needed) with no interference approximately 25 feet west of original tap location. New tap location Is in service road.

9/20 — contractor provided a proposed change order for anticipated extra work. Estimate was for $33,410.91. Subsequent correspondences between Wastewater
and Contractor lowered costs to $24,764.69 by having City do the pavement repair of service roadway.

Contractor stated in email that he wanted approval on cost proposal on 9/20 otherwise the subcontractor doing the earthwork will leave site for a few weeks.
Contractor also stated he has another job starting in mid October and needed to keep this job moving forward. Woodard Curran (consultant) recommended
issuing a work directive informing the Contractor to keep work moving forward while the Change Order is reviewed and costs substantiated. A work order for
contractor to continue working was issued on 9/20 pending authorization of City to allow for emergency change order approval.

Attached is TBuck’s latest cost proposal for CO2 in the amount of 524,764.69. It is being review by consultant Woodard Curran and will likely require several
correspondence back and forth between consultant and contractor to verify costs are legitimate.

Due to unforeseen circumstances and inaccuracies in the record drawings, additional work is required to complete this project. In an effort to avoid delays
and additional mobilization expense, the Wastewater Staff are requesting emergency approval of Change Order #2 to the contract with T-Buck Construction
PO # 146253 for the Water Booster Station in the amount of $24,764.69,

Please let me know anything additional needed from DPW to complete the processing of this Change Order
