2022 Sewer Replacement Project - SUR West Change Order No. |

To approve Change Order No. | to the 2022 Sewer Replacement Project with SUR
Construction West, Inc. of Winchester, NH for an amount not to exceed $500,000.00.
Funding will be through Department: 169 Wastewater; Fund: Bonded; Activity: Sewer
Repair.

2022 Sewer Replacement Project, Change Order No. 1

SUR Construction West was contracted for the 2022 Sewer Replacement work under
IFB0779-021022. SUR West has been our sewer contractor since 2020, and we have
found their work to be excellent and reasonably priced. Through completed Close Circuit
Television (CCTV) pipe inspection work, we have identified additional sewer pipe that
needs repair or replacement. The Engineering Department is finalizing plans for work on
Factory St and Lock St and would like to secure funding for construction this fall. The
purpose of this Change Order is to expand SUR Construction West’s contract to include
replacement of additional deteriorated sewer pipes.

The above change order will allow for repair or replacement of deteriorated pipe segments
and accompanying sewer structures and sewer services, if necessary. If portions of these
pipes are found to be in satisfactory condition, or suitable for lining, they will be retained
and funds used for repair or replacement of other sewer pipes and structures found to be
in disrepair through the City’s ongoing CCTV work.

Wayne G. Fillion, P.E. — President, MEP/FP Principal-In-Charge

Wayne is a registered Professional Engineer who has been with Yeaton Associates, Inc. for
nearly 30 years. He is the President of Yeaton Associates, inc., a position he has held for the
past 20 years. Wayne began his career as an HVAC Design Draftsman for two engineering
firms based in Burlington, Vermont: Wieman-Lamphere Architects and Jennison Engineering,
inc. Prior te his role as President for Yeaton Associates, Inc., Wayne served as a Senior
Project Engineer as well as Vice-President of the company.

Ryan C. Mealley, P.E., CEM. —Mechanieal Engineering Manager

Ryan is a registered Professional Engineer at Yeaton Associates, Inc. Prior to joining the
& company, Ryan held the title of Senior Mechanical Engineer at the engineering firms of AKF
hee Group and CES, Inc, as well a5 being the Mechanical Engineering Department Manager for
Diversified Technology Consultants in Connecticut. Ryan has extensive experience in
healthcare, academic, industrial, commercial and federal government facilities.

Lenny J. Edmunds — Electrical Project Manager

Lenny is an Electrical Project Manager at Yeaton Associates, Inc. Prior to joining the
company, Lenny held the position of Senior Electrical Designer at RFS Engineering in Laconia,
New Hampshire, and previous to RFS, was a 23 year veteran of C & M Engineering in
Manchester, New Hampshire. Lenny has extensive work experience in Educational,
Healthcare, institutional and Commercial Electrical Engineering. Lenny works out of the
Bedford, New Hampshire Office and resides in Concord.

Keith A. DeMoura — Construction Administrator

Keith is the Construction Administrator for Yeatorn Associates, Inc. He has been the
Owner/Operator of KND Metals for the past two decades servicing the greater New England
area, Keith has extensive HVAC experience in medical facilities, educational facilities, office
buildings, commercial buildings and industrial facilities.
