Vendor will complete the Work in accordance with the Contract Documents for the following prices:

A. Vendor acknowledges that estimated quantities are not guaranteed, and are solely for the
purpose of comparison of proposals, and final payment for all Unit Price Bid items will be
based on actual quantities provided, determined as provided in the Contract Documents.

The following documents are attached to and made a condition of this Proposal:

A. Bid Form —which includes a tabulation of Subcontractors, Suppliers and other individuals and
entities required to be identified in this Proposal;

and specifications and being familiar with all of the conditions surrounding the construction of the proposed

Qualification Statement: detailing experience on similar projects of scope, size & scale.

References may be contacted as part of our preliminary bid review to assist project committee in assessing
Vendor qualifications. Current City of Nashua personnel and elected officials shall not be used as references.

City Projects: Provide a list of project awards Vendor has received from the City of Nashua in the past 5 years.
Please include project detail, City department and City project lead.

Acknowledgement is made of receipt of the following addenda: N/A

If notice of acceptance of this proposal is given to the undersigned within ninety 90 days after the date of opening
of bids, or any time thereafter before this bid is withdrawn, the undersigned will execute and deliver a contract
in the prescribed form within 10 days after the contract has been presented to them for signature.

PROPOSAL QUALIFICATION STATEMENT

Detailed Plan:

All metal recycling will be processed at Windfield’s scrapyard located at 15 Medford St., Lawrence, MA.
Hours of Operation: Monday-Friday 7:30AM to 4PM and Saturday 7:30AM to 12PM Noon.

Windfield will provide 20yd, 30yd, and or 40yd roll-off containers as needed and upon request.

If live loads are requested by Four Hills Landfill Recycling Center, Windfield will provide trucking service
to do so if necessary.

Equipment includes: Sierra RP600 for prep of Light Iron, Sierra T55OSL 700 Ton Shear for preparing
Heavy Metals, Sierra HRB Bailer, Multiple Fork Trucks, State Certified Truck Scale, Sennebogen 835E, and
Fuchs MLH350 for material handling.

Windfield’s Scrap Right program will allow us to create monthly statements as requested.
Capacity of Material handling is 3000 Tons/Wk.

On our Windfield Alloy website, www.windfieldalloy.com, clients have the ability to request a pick-up or
swap of roll-off containers on our portal.

Description of Facility:
Windfield Alloy, Inc. was established in 1978. We have (2) facilities, Atkinson, NH and Lawrence, MA

Warehouse Location: Headquarters - 2 Route 111 Atkinson, NH 03811

Warehouse Size: 100,000 SF +/- and 4 Acres of parking/storage area

Areas of Interest: Electronics Recycling/Asset Management/Remarketing, Non-Ferrous Metals
Recycling, lron/Ferrous Metal Recycling, C&D Waste & Recycling Solutions, Cardboard, Plastics, Wood.

Logistics Experience: With national locations and its third-party logistics relationships, Windfield’s
service radius and transportation network extend nationally & internationally. This strategic and
geographically dispersed transportation network provides for timely pickup services that guarantee a
continuous movement of materials. Dedicated to providing the best service possible, Windfield works

Pursuant to NRO § 5-74 (B), which states that a contract that extends from the current fiscal year into
succeeding fiscal year(s) in which no funds have been appropriated nor otherwise designated for this
purpose shall be approved by the full Board of Aldermen before the contract shall become binding on the
City.

The Finance Committee approved and placed on file the notification of the award of the referenced contract
at their October 18, 2017 meeting. | am requesting the concurrence and approval of the full Board of
Aldermen for the award of the following contract:

NAMI New Hampshire (Connect Suicide Prevention Project) — Included is the Purchasing Manager’s Memo
#18-047 regarding the communication of this contract to the Finance Committee dated 10/12/17. This is a
two (2) year contract.
