CITY OF NASHUA REPRESENTATIVE The City of Nashua may designate a City of Nashua
representative for this contract. If designated, all notices, project materials, requests by Professional
Consultant, and any other communication about the contract shall be addressed or be delivered to
the City of Nashua Representative.

CHANGES TO SCOPE OF WORK The City of Nashua may, at any time, by written order, make
changes to the general scope, character, or cost of this contract and in the services or work to be
performed, either increasing or decreasing the scope, character, or cost of Professional Consultant's
performance under the contract. Professional Consultant shall provide to the City of Nashua within
10 calendar days, a written proposal for accomplishing the change. The proposal for a change shall
provide enough detail, including personnel hours for each sub-task and cost breakdowns of tasks,
for the City of Nashua to be able to adequately analyze the proposal. The City of Nashua will then
determine in writing if Professional Consultant should proceed with any or all of the proposed
change. If the change causes an increase or a decrease in Professional Consultant's cost or time
required for performance of the contract as a whole, an equitable adjustment shall be made and the
contract accordingly modified in writing. Any claim of Professional Consultant for adjustment
under this clause shall be asserted in writing within 30 days of the date the City of Nashua notified
Professional Consultant of the

change.

When Professional Consultant seeks changes, Professional Consultant shall, before any work
commences, estimate their effect on the cost of the contract and on its schedule and notify the City
of Nashua in writing of the estimate. The proposal for a change shall provide enough detail,
including personnel hours for each sub-task and cost breakdowns of tasks, for the City of Nashua
to be able to adequately analyze the proposal. The City of Nashua will then determine in writing if
Professional Consultant should proceed with any or all of the proposed change.

Except as provided in this paragraph, Professional Consultant shall implement no change unless the
City of Nashua in writing approves the change. Unless otherwise agreed to in writing, the provisions
of this contract shall apply to all changes. The City of Nashua may provide verbal approval of a
change when the City of Nashua, in its sole discretion, determines that time is critical or public
health and safety are of concern. Any verbal approval shall be confirmed in writing as soon as
practicable. Any change undertaken without prior City of Nashua approval shall not be
compensated and is, at the City of Nashua's election, sufficient reason for contract termination.

CITY OF NASHUA COOPERATION The City of Nashua agrees that its personnel will cooperate with
Professional Consultant in the performance of its work under this contract and that such personnel
will be available to Professional Consultant for consultation at reasonable times and after being given
sufficient advance notice that will prevent conflict with their other responsibilities. The City of
Nashua also agrees to provide Professional Consultant with access to City of Nashua records in a
reasonable time and manner and to schedule items that require action by the Board of Aldermen
and Finance Committee in a timely manner. The City of Nashua and Professional Consultant also
agree to attend all meetings called by the City of Nashua or Professional Consultant to discuss the
work under the Contract, and that Professional Consultant may elect to conduct and record such
meetings and shall later distribute prepared minutes of the meeting to the City of Nashua.

DISCOVERY OF CONFLICTS, ERRORS, OMISSIONS, AMBIGUITIES, OR DISCREPANCIES
Professional Consultant warrants that it has examined all contract documents, has brought all
conflicts, errors, discrepancies, and ambiguities to the attention of the City of Nashua in writing,
and has concluded that the City of Nashua's resolution of each matter is satisfactory to Professional
Consultant. All future questions Professional Consultant may have concerning interpretation or
clarification of this contract shall be submitted in writing to the City of Nashua within 10 calendar
days of their arising. The writing shall state clearly and in full detail the basis

All bidders must acknowledge receipt of this Addendum No. 1 by signing it and including with
the bid or stating “acknowledge Addendum 1” directly on the bid sheet. Bids submitted without
acknowledgement of receipt of this addendum may be considered non-responsive.

The following information and changes are now part of the Contract Documents.

DRAWING

1 Remove “New 3” Valve (Contractor to verify pipe size) for the valve into the primary
tanks. Add at two locations on north wall of primary clarifiers “Replace 2” Copper Plant
Water Valve in Pipe Gallery” (see revised drawing).

Note: the pipe gallery is a below grade concrete vault that runs the length of the north

face of the primary clarifiers with stairway entrances at both ends. It houses pumps,

pipes, controls and valves. There is plenty of work space and lighting available to work.

The existing copper (or brass) valves are 2-inch gate type that tee into the existing 2”

copper pipe with solder weld. The new valves will be 2-inch ball type (brass) valves to

be soldered to existing copper pipe.

Add “New 6” Valve “ to the 6” DI line just north of the Aeration Tanks (see revised

drawing).

3. Add electric and instrumentation control boxes that are visible and adjacent to new valve
and hydrant installation at 3 locations — hydrants 4, 5 and 14 (2 on the west face of
primary clarifiers and one at sodium hypochlorite storage tanks.

4, Add note “Cut 7’x7’ section of concrete slab & restore after installation of new valve &
hydrant” at hydrant no. 14 location of sodium hypochlorite storage tanks.

5. Add note “Remove riprap & restore after installation of new valve and hydrant” at hydrant
no. 17.

6. Eliminate note “Pipe freeze to replace valve & hydrant” at hydrant no. 19”. One of the
shut off valves will likely be used instead of the need to pipe freeze.

7. Add note “Existing plant water enters building 8’ below grade” at the location of the
inserta valve to be installed near the entrance to the Sludge Processing and Dewatering

GUESTIONS RECEIVED

I.

Will the Plant Electrician repair an electric or communications line accidently severed
during construction that is not marked in the field?

Answer: Yes, however the contractor must excavate in a slow and cautious manner. The

contractor should remove a small amount of material in each in each scoop, 1-foot or less
of depth. Additional hand digging or probing should be done in advance of excavation to
verify there are no unknown utilities present.

Shouldn't a 6-inch inserta valve be installed on the water line that feeds around the
primary clarifiers before installing the two 3-inch valves?

Answer: Yes, if there are sufficient funds in the project to cover this additional cost. If
not the City reserves the right to eliminate the additional 6-inch valve and use pipe
freezing methods to isolate the two 3-inch valves (if more cost effective). An additional
6-inch inserta valve was added to the bid sheet and is shown on the drawings.

Scope of Work — Valve and Yard Hydrant Installation

EXHIBIT C

There are 21 yard hydrants and gate valves at the Nashua Wastewater Treatment Facility located
at 2 Sawmill Road that are in need of replacement. The yard hydrants are on the plant water
systems or chlorinated effluent from the treatment plant. The existing plant water system was
originally installed in 1972 and later expanded in 1988 and again in 2000. The original plant
water system is cast iron pipe and in the later year’s ductile iron pipe was used. The pipe
material is shown on the plans. As a result of the age of the installation of the system, many of
the yard hydrant and associated valves do not operate properly.

The plant water system consists of effluent water that is filtered and chlorinated. It is pressurized
to 125 psi by four pumps in the lower level of the pump building. It typically pumps between
200-300gpm depending on the water use.

This project will occur in the 2 phases. First is the installation of the gates valves on the plant
water system mains at locations as shown in the Proposed drawing. The following size valves
will be installed using live without having to turn off the plant water: 1 — 12” valve, 1 — 10”
valve and 2 — 6” valves. In addition, 3 — 3” valves will be installed using pipe freezing
techniques to isolate the pipe at the locations shown on the Proposed Drawing. Each valve
installation shall include a valve box size for the installation (refer to specifications). Two valve
keys shall be supplied to match a standard 2-inch square top nut and long enough to extend a
minimum or 3-feet above ground. The contractor shall have a test pit dug at to verify the outside
diameter at four locations to verify the pipe size prior to ordering the valves. The test pits should
be dug at the same location where the valve will be installed. The depth is assumed to be 5— 6
feet. It shall be backfilled when done unless the contractor is able to get the valve within a few
days of the dig. Alternatively, the test pit may be plated over if it completely covers the
excavation. New gate valves should be installed on lawn areas (when possible) to minimize
disturbance to paved roadways. The number of gate valves to be installed may be limited by
funding or constraints that may be encountered prior to installation.

The second phase of the project will involve the installation of the 21 yard hydrants, valve shut
offs and connection piping/fittings. One hydrant was dug up recently (location number 6 on the
Design Plan) and the main was determined to be 4” (OD — cast iron) with a 4x4”x2.5” (all OD
measurements) tee. Based on this information, the contractor shall leave the main tee in place,
cut off the pipe at an appropriate distance from the tee and connect with a coupling to new pipe,
gate valve, yard hydrant and associated fittings.

The contractor will supply all coupling, pipe sections, gate valve shut offs, and yard hydrants.
and hydrant, gravel (min. 1 c.f.) for weep hole at base of hydrant and thrust block along with
general installation. Min bury is 5.5 feet above piping. The yard hydrants shall be #2 Post, 5’-6”
bury, 3” MJ inlet, duel outlet nozzles1-1/2”, NTS, ductile iron riser with cast iron top, galvanized
steel operating rod, self-draining, nonfreezing. The gate valves shall be 3” RW gate valve MJ,
open left, C509 Mueller or equal. Fitting are to be 3” EBBA DI Megalug and 3” MJ Gasket &
Bolt Pack. Valve box shall be a 2-piece slide type top flange valve box, complete with

“WATER” marked lid. Contractor is responsible to supply all miscellaneous piping, fittings and
other materials not mentioned above.

One yard hydrant and valve is missing (number 16 on the design plan). The contract will need to
dig at the approximate location marked in the field to locate the old hydrant stem and valve box.
This work is incidental to contract.

The number of gate valves and yard hydrants to be installed may be limited by the funding
available or constraints that may be encountered prior to installation. The City reserves the right
to eliminate yard hydrants at less critical locations depending on the outcome of the bid.

There are asphalt aprons around the primary and grit tanks that may be compromised during the
installation. It is assumed at these locations that a 1.5” thick patch will be sufficient. Asphalt
repairs at locations on. a main drive is impacted in around outside digester it should meet City
specs for a minor roadway or a minimum of 1’ gravel, 2” base and 1.5” top coat.

Sidewalk replacement shall be according to City details for sidewalk replacement. During
sidewalk repairs it will be necessary to remove and reset of curbing, which is incidental to the
sidewalk replacement.

All disturbed grass areas to be loam & seeded. All old yards and other debris shall be removed
by the contractor. Miscellaneous debris may be delivered at no additional cost. Delivery of
debris must be scheduled in advance with the Solid Waste Department and arrive at the facility
no later than 2pm.

Technical Specifications for Water Valve and Yard Hydrant Installation

General: These specifications are not necessarily inclusive of all the work being done. The
contractor shall submit shop drawings of all yard hydrants and valves, water main valves, piping
and all fittings to be used prior to starting any work.

1. Ductile Iron Pipe for Service Connection to Existing Pipe

A. New ductile iron water pipe is to be used from where the existing service line pipe is cut
to the location of the new yard hydrant being installed. All ductile iron pipe shall
conform to AWWA Standard C151.

B. If ductile iron pipe is not available for the service line pipe size, the contractor shall
submit a shop drawing of the pipe to be used. No PVC, plastic or copper pipe is
permitted.

C. The pipe shall have an exterior epoxy coating on it to prevent corrosion.

D. Contractor shall determine appropriate connection fitting from the existing pipe to new
ductile or iron pipe. All fittings shall be durable ductile iron or iron, epoxy coated and be
a megalug or other mechanical joint to firmly connect the two pipes.

2. Water Main Gate Valves

A. Water main gate valves for the 6-inch, 10-inch and 12-inch size shall be live tap valves
equal to Hydra-Stop Insta-Valve 250 Patriot Series.

B. Traditional line tapping methods shall be used for the Installation of all Insertion Valves
to allow removal of a single coupon for system evaluation. Reaming the pipe, complete
removal of a section of pipe (top & bottom), or milling a slot in the pipe shall be
prohibited.

C. All insertion valves must be capable of working on Cast/Grey Iron or Ductile Iron Class
A, B, C and D, IPS PVC, C900 and C909 PVC, Steel and AC pipe diameters without
changing either top or bottom portion of split valve body or using a transition gasket.

D. All insertion valves must provide a solid support of the host pipe through the entire
laying length of the valve body. No gaps or space between the valve body and host pipe
shall be accepted.

E. All insertion valves shall be rated for 250 psig maximum working pressure. The pressure
rating must be permanently marked into the body.

F. All insertion valves must be hydrostatically pressure tested to 1.25 times of the system
operating pressure (minimum) or 1.5 times of the Insertion Valves 250 psig maximum
pressure rating. The test shall be sustained for a minimum of 15 minutes. Once the
pressure test is affectively achieved the insertion valve body must not be moved in
accordance with AWWA Standards. If the insertion valve body is moved the pressure test
