FY17 Paving Program — Construction

To approve the award of the construction contract for the FY17 Paving Program to
E.J. Paving Company, Inc. of Methuen, MA in the amount of $1,729,148.30.
Funding will be through: Department: 161 Street; Fund: General; Activity: Paving;
Department: 161 Street; Fund: Trust; Account Category: 54 Property Services;
Department: 169 Wastewater; Fund: Wastewater; Activities: Sewer Structures and
Sewer Rehab.

FY17 Annual Paving Program Street Paving List

The FY17 Paving Contract is expected to pave approximately 5.6 miles of City streets
including 2,500 feet of Main Street and 3,100 feet of West Hollis Street. The contract
uses repair methods such as shim, overlay, milling and reclamation. It also includes
replacing obsolete and broken drainage and sewer castings.

The contract was advertised on February 3, 2017. Six contractors attended the
mandatory prebid and three contractors submitted bids on March 2, 2017. Bids are as

follows:

E.J. Paving Company Inc. of Methuen, MA $1,729,148.30
Brox Industries, Inc. of Dracut, MA $1,796,727.80
Newport Construction Corporation of Nashua, NH $1,900,519.10

E.J. Paving Company, Inc. of Methuen, MA at a bid price of $1,729,148.30 is the
qualified low bidder.

CONSTRUCTION NOTES:

1. CONSTRUCT THIS PROJECT INACCORDANCE WITH THE MORE STRINGENT OF THE
NHDOT STANDARD SPECIFICATIONS FOR ROAD AND BRIDGE CONSTRUCTION (LATEST
APPROVED EDITION), OR CITY OF NASHUA, DIVISION OF PUBLIC WORKS STANDARD
PLANS, SUPPLEMENTAL SPECIFICATIONS AND SPECIAL PROVISIONS.

2. OVERHEAD AND UNDERGROUND ELECTRICAL AND COMMUNICATION LINES ARE
LOCATED THROUGHOUT THE PROJECT WITH CROSSINGS AT VARIOUS LOCATIONS.
EXTREME CAUTION IS REQUIRED IN THE OPERATION OF CONSTRUCTION EQUIPMENT.

3. EXACT LIMITS OF PAVE AND LOCATIONS/DEPTHS OF GRINDING WORK ARE PER THE
ENGINEER.

4. UNLESS OTHERWISE DIRECTED, USE THE MIXES WITH STONE SIZES AS SHOWN ON THE
TABLE OF THE DIMENSIONS SHEET.

5. REPLACE EXISTING STRIPING AS SOON AS PRACTIBLE FOLLOWING REMOVAL OR
ABANDONMENT OF THE EXISTING.

6. THE WORD "CASTING" IMPLIES BOTH FRAME AND GRATE FOR CATCH BASINS AND
FRAME AND COVER FOR MAN HOLES.

7. ADJUST ELEVATION OF EXISTING CASTINGS (TYP.) REPLACE TRIANGULAR CASTINGS
(TYP.) REPLACE ROUND CASTINGS AND CATCH BASIN CASTINGS AS DIRECTED BY THE
ENGINEER.

8. ALL FILLETS SHALL BE TEMPORARY, REMOVED AT TIME OF PAVE AND INCIDENTAL TO
GRINDING ITEM. ALL FILLETS SHALL SPAN THE FULL WIDTH OF ROAD OR DRIVEWAY.

9. CONTRACTOR IS RESPONSIBLE FOR PROVIDING SHOULDER GRAVEL, OR LOAM AND
SEED IN PLACE AS SOON A PRACTICABLE AFTER OVERLAY.

10. CONTRACT WORK SHALL INCLUDE THE TREATMENT OF SIDE STREET APRONS TO THE
EXTENT DIRECTED BY THE ENGINEER.

11. INSTALL WARNING SIGNS FOR APPROACHING TRAFFIC: "BUMP", "GROOVED PAVEMENT".
MARK JOINTS AND RAISED STRUCTURES WITH REFLECTIVE PAINT TO BE MAINTAINED
UNTIL FINAL OVERLAY IS COMPLETED.

12. ROAD AND LANE CLOSURES SHALL BE IMPLEMENTED ACCORDING TO MUTCD AND
NHDOT STANDARD SPECIFICATIONS, AND RECEIVE REQUIRED CITY OF NASHUA
PERMITS.

13. CONTRACTOR SHALL RE-ESTABLISH ALL EXISTING DRIVEWAY BERMS AND WHERE
PRACTICALBE MAKE THEM INTEGRAL WITH THE OVERLAY TREATMENT.

14. CONTRACTOR SHALL CONTACT DIG SAFE AND FOLLOW ALL PROCEDURES.

15. ANY PIPE WORK WITHIN THE CITY'S R.O.W SHALL BE PERFORMEND BY A CITY OF
NASHUA LICENSED DRAIN LAYER.

For Professional Services for Treatment Works
I. Services to be Performed by the ENGINEER

A. Upon execution of this AGREEMENT, the ENGINEER shall proceed with the general administration of construction
and fulltime inspection by qualified personnel of the contractor's work to assure compliance with the contract
documents and any coincident or subsequent changes or change orders for the construction of the treatment
works for the (Fewnr/City) of Nashua, New Hampshire, Sewer Siphon Rehabilitation. Said services shall include,
but shall not necessarily be limited to:

1. General Construction Administration
a. Updating and modifications of contract documents to meet changed site and project conditions or
variations in State or Federal requirements.

Completion of formal contract documents for the award of contracts.

Checking detail construction, shop and erection drawings submitted by the contractor.

Reviewing laboratory, shop, and mill test reports of materials and equipment.

Preparation of drawings and technical material as required to supplement and/or clarify the contract

documents.

g. Review, verification, and approval of estimates for periodic and final payments to the contractors for
submittal to the OWNER and the DIVISION.

h. Periodic inspection of work and final inspection.

i. Observing and reporting performance and qualifying tests required by specifications.

j. Keeping daily records of construction progress and compiling same into progress reports for submission
to the OWNER and DIVISION.

k. Verifying and keeping records of construction items of work accomplished for use as a basis of checking
contractors’ monthly estimates.

|. Consultation and advise during construction.

m. Determination of need for and preparation of change orders for approval by the OWNER and DIVISION
based on conditions found during construction and/or additions or modifications to the work requested
by the OWNER and approved by the ENGINEER and the DIVISION and implemented at a price
recommended by the ENGINEER as equitable. For change order items exceeding $10,000, the ENGINEER
shall prepare an independent cost estimate.

n. Determination of “equality” for substitution of materials and equipment specified and securing DIVISION
approval thereof.

o. Provide and maintain construction control lines and grades. For sewers offset lines and grade elevations,

with cut depths at manholes; at treatment plant and pumping station sites base lines for layout and

q. Preparation of certificate of substantial completion and contract completion.
r. Assist the OWNER in obtaining the required project related approvals of State and Federal agencies.
