Ms. Parkinson,

The Information Technology Department needs to upgrade the City’s Risk Management software
from RiskMaster 7.1 — client hosted, to Assure Claims — SaaS hosted (Computer Sciences Corporation
is the vendor for both products).

The reason for the upgrade is due to the following; SQL 2003 server that the operating system
resides on is no longer supported by Microsoft, the XP operating system that the client is installed
on is no longer supported by Microsoft, and the RiskMaster 7.1 version is no longer supported by
the vendor. Please refer to Work Order No.5 (attached) for a detailed cost break down for the
upgrade.

The Information Technology Department recommends the upgrade performed by Computer
Sciences Corporation in the amount of $39,232.00.

Funding though the following accounts: 22.1010 — 53142: $29,512.96, and 122 Information
Technology — Consulting 22.1.500 — 53142: $9,719.04

This Work Order is effective March 8, 2022, and is hereby made a part of and incorporated into the
Master Services Agreement No. 01-STE-0106 dated June 28, 2002 and any Addendums to Agreement No. 01-STE-
0106 (“Agreement”) by and between Computer Sciences Corporation, a DXC Technology company (“CSC”) and City

Assumptions:
1. User will manually enter the needed input fields in the dashboard prior to running reports.
2. The csv/Excel files will contain headers. User must manually make format changes as needed to the files
and manually rename as needed.
3. All data must exist in Assure Claims.
4. Custom changes will need to be made in Elasticube to support case statements/conditional logic.
5. Estimate is for designing 2 dashboards with filters without any specific formatting (based on specs and
examples already provided to CSC).
6. With Sisense option Customer may need to manually update or rename the file. Through the utility, the file
can be coded to format and named as per requirements.
7. Work Order is Time and Materials, billable upon delivery of Services.
8. The Parties hereby agree that Services set out in this Work Order No. 5 shall not commence until Work
Order No. 6, “Assure Claims Access Work Order” is fully executed by CSC and Customer.
Notes:
e Explorer is out of scope.
e New Sisense Reporting is priced and is needed moving forward.
e Sortmaster is not supported.
e If Customer universe is over 5 years old, additional hours may be needed via a PCR.
e Assumes no customizations.
e Timing would be 7 to 10 weeks for the required new server to replace all Windows 2012 and 16 servers.

Training Assumptions

Training estimates are for a maximum of 12 participants per class except where noted otherwise.

A day of training consists of 6 hours of training regardless of whether training is conducted at Customer's
site or remote via webinar.

Training participants are employees of Customer’s organization. Should customer desire to include any non-
employees, such as contractors or consultants, Customer will notify CSC of this at least two weeks prior to
the scheduled start of the first training session. This will allow time to prepare a non-disclosure agreement
(NDA) with any contractors or consultants.

Customer daily-rate training includes pre- and post-training activities and associated project management.
All training classes are quoted using a train-the-trainer approach: CSC trainers will train selected Customer
team members who will then train the remaining Customer staff. Should Customer decide to have CSC train
all personnel directly and there are more than 12 participants, additional charges will apply.

Assure Claims End User Training

Assure Claims End User training includes training on Assure Claims basics on the lines of business needed.

Sisense Reporting Assumptions

Sisense End User training includes training for those who will be creating and modifying dashboards.
Sisense System Admin training includes training for those who will be administering the Sisense system. It

covers topics such as adding and managing users and maintaining elasticubes.

To allow for an integration of Sisense dashboard within the UX version of RISKMASTER/Assure Claims,
Customer must PURCHASE and enable single sign-on (SSO).

Single sign-on (SSO) is required if Customer wants to see a Sisense Dashboard within RISKMASTER/Assure
Claims, otherwise SSO is not required.

Linux server is required for on-premise. Customer is opting for SAAS AWS so this is not required.

Configuration Training Assumptions

Assure Claims Configuration training should always be done at Customer's site and not via web session.
Assure Claims Configuration training includes training on system configuration and customization options
as well as training on system administration and maintenance activities. This should be conducted with no
more than 5 individuals in the training class.

Assure Claims Configuration consulting provides as-needed assistance with configuring the Assure Claims
system to reflect desired business processes. It does not include CSC staff completing the actual
configuration.

Data Services Assumptions

Duplicate Employee Names: A list of duplicate employees can be extracted and the duplicate record marked
as soft deleted in Riskmaster so it won’t impact any existing claim/event. New claims entered in Riskmaster
will not see duplicate employees while selecting an employee.

Employee “Department” - Customer should provide a list of employee numbers along with their correct
department. CSC will write a SQL script to attach the employee with the required correct department.
Customer will provide a copy of its Riskmaster production database to start the data cleanup process in CSC
development environment.

Customer will provide Riskmaster database to CSC via SFTP.

CSC will write SQL scripts to clean up the data in Customer’s existing Riskmaster database.

Any changes needed to the existing BI reports is out of scope. Customer has the responsibility to update its
existing BI reports with these changes as needed.

CSC will soft delete the duplicate employees. Criteria for duplicity will be same employee number, name,
and address unless customer wants to add or exclude a name and address. Legacy data will not be updated.
Customer will ensure that the ‘manual weekly upload’ of employees matches the existing records on
employee numbers to maintain uniformity. The weekly upload should not create a new employee if the
employee number exists already in Riskmaster. It should also ensure that if it is updating the employee
department that the correct employee department should be updated by the weekly upload. This activity
is out of scope of this Work Order.

There will be two (2) cycles of delivery: first Test/Training environment, and then Production. Should
Customer want additional delivery, the additional delivery hours will require a change control order and be
subject to additional fees.

Database Analysis Assumptions

Customer will share or provide a copy of its RISCMASTER Production database, View database, Session
database, TaskMaster database and Security database.

Database analysis will be performed to ensure Customer’s environment after upgrades are stable. The
process involves comparing the upgraded Customer database with the standard Riskmaster database of a
new Riskmaster environment and to identify any gaps.
Gaps include the following:

o The information of new tables in Riskmaster database.

o The information of new columns in database tables.

o Mismatches in tables column Data type and other column properties (e.g., sizes).

o Archival information for descoped tables and columns, etc.
All gaps are documented and thereafter necessary SQL scripts are created to address all identified gaps.
These SQL scripts are delivered as an outcome of Database analysis along with instructions and sequence
for executing these scripts.

Upgrade Lawson Employee to Assure Claims Interface

Customer will purchase DA DIS licensed tool to be used for importing employee data into Assure Claims.
Customer will provide the source data via SFTP. The entirety of the source data will be provided to start the
data import process.

CSC will write batch jobs for reading the source data and populate it into MS Access database instead of the
old MDB database and will then use the DA DIS tool for importing data from the MS Access database to
Assure Claims.

CSC will use the same employee matching criteria as is used currently and setup the periodic import as per
current the current frequency.

Based upon the employee matching criteria, DA DIS will either create new employees or update existing
employees in Assure Claims.

There will be two (2) cycles of delivery: first the Test/Training environment, and then Production. Should
Customer want additional delivery, the additional delivery hours will require a change control order and be
subject to additional fees.

Risk to Lawson Indemnity /Risk to Lawson AP
Assumptions:

User will manually enter the needed input fields in the dashboard prior to running reports.

Nightly Backups

Upon request, a nightly backup of the Customer database can be placed into SFTP for Customer to
download. Customer can restore the backup to a local Microsoft SQL Database Server. Customer
acknowledges that this data would not be real time, and any costs shall be solely borne by Customer. There
are no additional charges from CSC for this service.

Non-Disclosure Assumption
e This proposal includes data that shall not be disclosed outside Customer and shall not be duplicated, used
or disclosed — in whole or in part — for any purpose other than to evaluate this proposal. If, however, a
contract is awarded to this offer or as a result of — or in connection with — the submission of this data,
Customer shall have the right to duplicate, use, or disclose the data to the extent provided in the resulting
contract. This restriction does not limit Customer’s right to use information contained in this data if it is
obtained from another source without restriction. The data subject to the restriction is contained in sheets
of this entire proposal.

Note 1: Customer will pay all reasonable travel-related expenses incurred by employees of CSC in connection with this
project. Customer will pay one-way travel time for CSC personnel in fulfilling time and material tasks at the
hourly rate of $165.00 per person hour.

Note 2: Customer acknowledges that the above amounts are good faith estimates based upon the information known
to CSC. Estimates are not intended as price or performance guarantee. Number of hours is estimated for
budgetary purposes only. The estimate should not be deemed an absolute cap nor does it transform this
scope of work into a fixed fee agreement.

Note 3: In the event Customer cancels or reschedules a services appointment (e.g. including but not limited to
training or installation) with CSC, within seven (7) days of the scheduled date, Customer shall pay CSC a
cancellation / rescheduling charge equal to eight (8) hours of services, plus any non-refundable expenses
(including but not limited to airfare, hotel, car rental etc.) incurred by CSC.

Note 4: Authorized Location: 229 Main Street, P.O. Box 2019, Nashua, NH 03060.

Note 5: In the event Customer requires purchase orders, a copy must be provided promptly after the contract is
executed in order to include the associated purchase order number on each Customer invoice.

Note 6: In the event Customer is tax exempt, a copy of Customer tax exemption certificate may be required to
ensure that taxes are not applied to certain services.

Re: Contract for Aquatic Invasive Species Control and Management

Nuisance aquatic invasive plants are present in multiple Nashua water bodies, most notably the
Nashua River upstream of Mine Falls dam, the Mill Pond and Nashua Canal. During summer
2017 the City also received several complaints regarding excessive plant growth in Sandy Pond
and the Waterways Manager documented that Curly Leaf Pondweed, an aquatic invasive species,
was present in this pond. Aquatic invasive weeds, such as milfoil, fanwort, and water chestnut,
can make rivers and ponds unsightly, recreation unpleasant, and navigation by boat difficult.
Furthermore, excessive growth of vegetation can impair water quality due to reduced dissolved
oxygen which can cause fish kills.

The City’s Waterways Manager works closely with the State Invasive Species Coordinator at
NH DES to monitor the presence and extent of invasive species and update the Long-term Exotic
Aquatic Plant Management Plan for our waterways.

The City has been involved in controlling aquatic invasive species since 2011 when action was
taken to control Water Chestnut by mechanical harvest. This was very effective at removing
Water Chestnut and we now can continue to control Water Chestnut by hand pulling with
volunteers. Unfortunately, the decline in Water Chestnut created an opportunity for other
invasive species present in the River to take over. The vegetation is now dominated by milfoil
and fanwort which cannot be mechanically harvested or hand pulled, leaving herbicide treatment
as the only recommended effective management strategy. Herbicide treatments in rivers and
ponds are closely regulated and permitted which involves extensive public notice.

Herbicide treatments have been used in the Mill Pond and Nashua Canal every other year to
control primarily milfoil and fanwort since 2012. Herbicide treatments cannot eradicate these
plants therefore, continued periodic herbicide treatments are necessary to knock back the growth
of these invasive species.

For the last several years, herbicide treatments were used during the summer to control milfoil
and fanwort on 90 acres of the Nashua River upstream of the Mine Falls dam. This resulted in a
very noticeable improvement over 2016 conditions and we received multiple messages from
constituents reporting they were pleased with this outcome. Herbicide treatments are
recommended again this summer to continue reducing the coverage and extent of these invasive
species in this section of the Nashua River.

In response to complaints about the excessive plant and algae growth in Sandy Pond we will
continue taking action to control curly leaf pond weed and reduce nuisance algae to improve the
appearance and water quality of this pond.

Solitude Lake management is the only company in the region that provides this type of service,
and in accordance with the NH DES invasive species management grant program, we solicited a
proposal directly from them to complete the recommended management actions for the Nashua
River, Millpond and Canal, and Sandy Pond. The City was awarded a grant from NH DES in the
amount of up to $75,850. The total cost for this contract is $155,490 which will be paid in part
by the NH DES grant with remaining cost covered by Community Development (Nashua River),
the Mine Falls Trust (Millpond and Canal), and the Parks and Recreation Department (Sandy
Pond).

The Community Development Department recommends awarding the contract for aquatic
invasive species control in the Nashua River to Solitude Lake Management in the amount of
$155,490.
