Siphon Cover Modifications Work Scope and Bid Sheets

The City of Nashua ts seeking to upgrade sewer system siphon covers al various locations throughout the City.
Bids are being sought from qualified contractors for the installation of 2 new manufactured structure covers
with internal safety grate at one location and 10 other locations where an internal safety grates will be installed.
Details of the covers and safety grates to be installed are provided in the table below. Also attached is a location
map of the sites.

Vendors are encouraged to submit questions via e-mail; however, the City assumes no liability for assuring accurate and
complete email transmission/receipt and is not responsible to acknowledge receipt. Inquiries must be submitted in
writing, citing the RFP title, RFP number, Page, Section, and received no later than Wednesday, November 30, 2016

Provide coverage for not less than the following amounts or greater:

> General Liability: $1,000,000 per Occurrence
$2,000,000 Aggregate

* Motor Vehicle Liability: $1,000,000 Combined Single Limit
Coverage must include all owned, non-owned and hired vehicles.

* Workers’ Compensation Coverage according to Statute of the State of New Hampshire:
$100,000/$500,000/3100,000

ARTICLE 16— DISPUTE RESOLUTION

Upon receipt of notice of termination for cause, CONTRACTOR shall:

Immediately discontinue work on the date and to the extent specified in the notice.

2. Provide the OWNER with a list of all unperformed services.

Place no further orders or sub-contracts for materials, services, or facilities, other than as

may be necessary or required for completion of such portion of work under the contract

that is not terminated.
