You are directed to make the following changes in the Contract Documents:
Description: After the execution of a 3-year contract with Chemserve Environmental, the testing scope of
work changed due to increase testing requirements, established by New Hampshire Environmental
Services (NHDES) went into effect, especially for Per and PolyFlucroAlkyl Substances (PFAS) Testing.
Justification: In order to complete the remainder of FY21 required Groundwater and Leachate Sampling;
an additional $29,888 will be needed.
e Chemserve Environmental has submitted a revised timeline scope of work, which is attached and
made part of this Change Order, in the amount of $19,118.
e Additional funds are also necded to make payment on the balance of an invoice duc to Chemserve
Environmental, in the amount of $10,770.
