AGREEMENT BETWEEN THE CITY OF NASHUA AND

THIS AGREEMENT, effective this 1‘ day of January 2018, is made by and between the
City of Nashua, the address of which is 229 Main Street, Nashua, New Hampshire 03060
(herein called the “CITY”) and Revive Recovery, Inc., 263 Main Street, Nashua, New
Hampshire, 03060 (herein called the “RECIPIENT”).

WHEREAS, the CITY wishes to have the RECIPIENT provide certain services in the
community.

NOW, THEREFORE, the parties hereto do mutually agree as follows:
L SCOPE OF SERVICE

The RECIPIENT shall, in a satisfactory and proper manner, as determined by the CITY,
perform certain services as described in APPENDIX A, Scope of Services, attached hereto
and made a part hereof, generally described as employing a full-time Community Recovery
Coach.

ii. TIME OF PERFORMANCE

The RECIPIENT agrees that the funded activities, described in APPENDIX A, Scope of
Services, shall be performed between January 1, 2018 and December 31, 2018.

Hii, COMPENSATION AND METHOD OF PAYMENT

A. Amount of Grant
It is expressly agreed and understood that the total amount to be paid by the CITY to the
RECIPIENT under this Agreement shall be Forty Two Thousand dollars ($42,000.00).

B. Payments

The CITY will make equal quarterly payments to the RECIPIENT on or around January 1,
April 1, July 1, and October 1. Funds shall only by used for the salary and related
employment expenses of a Community Recovery Coach as described in APPENDIX A,
Scope of Services.

IV. SUBMITTALS & REPORTING REQUIREMENTS

The RECIPIENT must provide the CITY with monthly and quarterly reports containing de-
identified, aggregate information on the activities of the Community Recovery Coach.
Such reports shall, at a minimum, include data on the number of clients served, contacts
with the clients, and the services provided to those clients. Initial intake data, client
outcomes, and challenges will also be collected and reported, but without identifiable client
information. Upon receiving each report, the CITY may require the RECIPIENT to alter
the report, or collect different or additional data that would be more useful to the CITY.
