RESOLUTION

AUTHORIZING THE CITY OF NASHUA TO PARTICIPATE IN THE NASHUA
REGION INTEGRATED DELIVERY NETWORK (IDN) OF THE BUILDING
CAPACITY FOR TRANSFORMATION, A DELIVERY SYSTEM REFORM
INCENTIVE PAYMENT (DSRIP) PROGRAM

CITY OF NASHUA

In the Year Two Thousand and Sixteen

WHEREAS, the Centers for Medicare and Medicaid Services has created the Section
1115(a) Medicaid waiver, known as the Delivery System Reform Incentive Payment (DSRIP)
Program or "Building Capacity for Transformation" waiver,

WHEREAS, the State of New Hampshire (State) has made performance-based funding
available to regionally-based Integrated Delivery Networks (IDNs) that serve Medicaid
beneficiaries with the goal of transforming the delivery system for beneficiaries with mental
health conditions and/or substance use issues, including opiate abuse. The transformation will be
supported by participation of IDNs in Alternative Payment Models (APMs) that move Medicaid
payment from primarily volume-based to primarily value-based payment over the project period.

WHEREAS, the waiver allows for the State to invest $150 million over the next five
years to transform the State's behavioral health delivery system to improve care and slow long-
term health care costs. The goal of the statewide effort is to better support people on Medicaid by
transforming the behavioral health system by building clinical, inpatient and outpatient treatment
and community services capacity and integrating care and facilitation of transitions in care.

WHEREAS, IDNs are made up of multiple partners and stakeholders that represent
community-based social services agencies, support organization, hospitals, county facilities,
health care providers, mental/behavioral health providers, and public health that will partner to
design and implement projects to build capacity, to promote integration of primary care and
behavioral health services, to facilitate transitions in care, and to prepare for alternative payment
models.

WHEREAS, each IDN is mandated to implement a health care work force capacity and
health information technology development project that will deliver a core competency project
focused on the alignment of medical, behavioral and social service in an integrated model. The
IDNs are organized into seven regions across the state. This work of this project will focus on
developing the IDN workforce capacity to meet the project deliverables in the Greater Nashua
Region serving 19,110 Medicaid members.
