Summary of proposed materials and equipment (manufacturer, catalog number or
model, and description) and method of application or use, including but not
limited to encapsulants, wetting agents, personal protective equipment, and test
samples of all proposed materials for performing the Work. This includes, but 1s
not limited to copies of DOP certifications for negative air machines in
accordance with Subsection 2.02.A of this Section (Submittal #6).
A plan describing the method for exposure monitoring to document compliance
with OSHA Asbestos Standard, including frequency of monitoring, equipment
and calibration methods, and name of analysis laboratory (Submittal #7).
Lists of all permits, licenses, or manifests required to perform the Work specified
herein, including, but not limited to, those detailed in Subsection 1.08 of this
Section, but not including disposal site licenses covered in Submittals #9 and #11
below (Submittal #8).
Copies of the completed U.S. EPA, State, and local asbestos removal pre-
notification forms required to perform the Work specified herein, including, but
not limited to, those detailed in Subsection 1.08 of this Section (Submittal #9).
Copies of all proposed daily inspection and record logs, including Work Area
entry data, respirator inspections and maintenance, HEPA-exhaust inspections and
maintenance, and other work applicable activities including, but not limited to,
those detailed in Subsection 1.06 of this Section (Submittal #10).
Waste Disposal Plan (Submittal #11):
a. Submit a detailed plan for the preparation, handling and use of Waste
Shipment Records and or manifests in accordance with Subsection 3.08.1.
b. Name, location, and applicable licenses for primary and secondary
landfills for disposal of asbestos-containing or asbestos-contaminated
waste. Submit copy of required licenses and/or operating permit for all
disposal facilities including those approved by the DEP or other state or
federal regulatory agencies to accept non-friable asbestos-containing
materials as construction and demolition (C&D) waste. Provide a copy of
applicable federal and state permits for transporters of asbestos waste
materials.
C. Name and qualification of Contractor’s Authorized Agent for purpose of
signing the Operator’s Certification on Waste Shipment Records.
d. Submit to the Engineer, as part of Submittal # 11, letters of acceptance
from disposal facilities for the disposal of ACM or Asbestos-
Contaminated Waste Material. The letters of acceptance shall state that
