service, property owner of record, contractor, and nearest hospital posted at the
job site. The plan, at a minimum, must contain:
e A clear description of the project to be undertaken. Description should
include location, amount and type/types of asbestos material to be dealt with,
any site-specific conditions, and the anticipated start and finish dates.
e Identification of entity that will be performing the asbestos abatement
activities. The name of the project supervisor, copies of licenses, training
certificates, medical records, and respirator fit tests for those involved with the
asbestos project.
Definitive statements about the use of personal protective equipment.
A description of the type/types of air monitoring to be performed at the site.
Provisions for site security.
Specific information on the asbestos waste packaging, handling, and disposal
procedures. This must include the name, location, and permit of the asbestos
disposal site.
e Details of project completion requirements including, but not limited to, site
inspection, documented air monitoring results, documented soil analysis, as-
built drawings, record drawings, landfill receipts, and waste shipment records.
In addition the plan shall include: the sequencing of asbestos work; detailed
schedules by dates, shift times, and work activities during that shift; the interface
of trades involved in the performance of Work; methods to be used to assure the
safety of building occupants and visitors to the site and a detailed description of
the methods to be employed to control pollution. All work methods proposed to
execute the Work must be completely described in sufficient detail to document
that the work methods comply with applicable regulations. The Contractor shall
revise the original plan and re-submit to the Engineer for review in the event that
the original methods proposed are found to be ineffective. This plan shall be
prepared by an individual who has been trained in the discipline of an Asbestos
Project Designer. The Engineer's review of Submittal #4 will be general and does
not relieve the Contractor of responsibility to perform the Work using means and
methods that are in compliance with governing regulation (Submittal #4).
Contingency Plan: Submit a contingency plan for emergencies including fire,
accident, power failure, negative air system failure, supplied air system failure, or
any other event that may require modifications or abridgment of decontamination
or Work Area isolation procedures. Include in the plan specific procedures for
decontamination or Work Area isolation. Note that nothing in this Specification
should impede safe exiting or providing of adequate medical attention in the event
of an emergency. Include telephone number and locations of emergency services
including, but not limited to, fire, ambulance, doctor, hospital, police, power
company, telephone company (Submittal #5).
