Summary of workforce by disciplines, and a notarized statement documenting that
all proposed workers, by name, have received all required medicals and have been
properly trained in asbestos removal work, respirator use, and appropriate State of
New Hampshire, EPA & OSHA standards. The Contractor shall submit to the
Engineer the following items at least three days prior to individual workers being
allowed to work on the project (Submittal #2B):

a. Submit a copy of the training certificate and State of New Hampshire
certification for each worker.

b. Report from Medical Examination: Submit a copy of Physician's Written
Opinion as defined by 29 CFR 1926.1101(m)(4) for medical examination
conducted within last 12 months as part of compliance with OSHA
medical surveillance requirements for each worker who is to enter the
Work Area.

C. The Contractor should be aware that the State of New Hampshire has
indicated that asbestos-containing materials manufactured by Johns-
Manville not typically encountered during asbestos abatement projects
may be present beneath grade. A document is available entitled “Best
Management Practices on Johns-Manville Waste”. This publication is
available, free of charge, from the New Hampshire Department of
Environmental Services. The Contractor shall submit certification that the
Contractor is aware of the potential presence of this material and that the
Contractor has been notified as to the availability of this document.

Notarized Certification: Submit certification signed by an officer of the asbestos
abatement contracting firm and notarized that exposure measurements, medical

surveillance, and worker training records are being kept in conformance with 29
CFR 1926 (Submittal #3).

Plan of Action & Standard Operating Procedures: Submit a general plan of the
procedures proposed for use in complying with the requirements of this
Specification. If undocumented ACM is encountered during the Work, the
Contractor shall be required to submit a supplemental or revised site specific Plan
of Action. This detailed Site Work Plan (SWP) must be submitted to the Solid
Waste Compliance Section (SWCS) of the Waste Management Division of the
New Hampshire Department of Environmental Services for approval prior to
remediation of asbestos disposal sites or outdoor asbestos renovation, demolition,
or remediation projects. The SWPs, after being approved by the SWCS, must be
required reading for all personnel connected with asbestos abatement activities,
and must be posted at the job site. There must be a list of emergency telephone
numbers including, but not limited to, the local fire department, ambulance
