B) Project Safety Plans — An individual contract/ project specific safety plan will be
developed and used on each individual project undertaken by Sealcoating, Inc. The
plan will be developed based upon provisions contained in company safety programs
and procedures as well as the unique contract specifications and other project
specific requirements. Each project specific safety plan is to be prepared prior
to undertaking any project activity and is to be reviewed and approved by the
corporate safety director. Elements of the plan will consist of but not be
limited to:

e Project organization

Roles and responsibilities

Emergency action planning

Task identification

Hazard evaluation

Corporate disciplinary policy

General safety requirements

Safe work practice methods

Reporting and documentation requirements

C) Job Hazard Analysis Planning — In order to address high hazard situations which
may arise associated with undertaking either unique or routine project activity, the job
hazard analysis planning system will be employed an as needed basis to assist in the
identification of task hazards and the appropriate methods of elimination or control. A
standardized process will be employed in which the task steps will be identified, the
associated hazards will be determined, the preferred elimination or control method will
defined, and the individual within the company organization with responsibility to assure
successful implementation is recognized. This process will be documented and modified
based upon changes to task activity or conditions. The initial or modified document will
be used to undertake training of personnel to engage in the task prior to undertaking
the task and retraining will occur if any additional changes to the document happen.

4.0 FUTURE MODIFICATION PROCEDURES

The Sealcoating, Inc. safety program is designed to be a dynamic process that will adjust to
changes in the corporate scope or mission. Periodic evaluations will be undertaken to assess if
the program addresses the needs of the company and if formal changes to processes or
procedures need to be undertaken. This section provides a brief description of the evaluation
and modification process.

An evaluation of the Sealcoating, Inc. safety program effectiveness will be undertaken on a
scheduled basis to determine if modifications are required. The assessment process will be
directed by the corporate safety director and will be reviewed by senior management. This
review will consist of the evaluation of the program elements and overall safety performance of
the company through the evaluation period. Based upon review of performance reporting items
as well as other indicators, adjustments and or addition to program elements may be
recommended. Any changes to the program plan will then be agreed to and committed to by
senior management and a revised safety program plan will be prepared and issued to all
affected entities. This assessment and modification process may also be undertaken at more
frequent intervals or on an as needed basis if corporate conditions arise that warrant the
