CHANGE ORDER (ABSESTOS ABATEMENT)
1. Proposed change:
The following is a change to the original contract to include the removal of
asbestos. All State Abatement Professionals, INC will be performing the
asbestos abatement to include the following:
Preparation and isolation of work area
Installation of engineering controls and decontamination system
Asbestos abatement of asbestos roofing materials
Decontamination of all surfaces
Encapsulation of all surfaces with lock-down encapsulation
Final air clearance test
The Nashua Police Department must provide all electrical and water
requirements for debris removal
e ASAP will make all necessary notifications to all local, state and federal
regulations
* Upon completion of the asbestos removal, ASAP all documentation will be
submitted to the City of Nashua for the assurance against and legal
entanglements or litigations
e This includes the additional labor for Chick Beaulieu Inc to install the
new rubber roof system.
2. Note:
e This change order becomes part of and in conformance with the existing
contract.
We hearby agree to the above change in accordance with the above
specifications for the additional cost of:
Thirty Four Thousand Two Hundred Fifty 00/100 Dollars
($34,250.00)
Total project cost to be $113,150.00.
