This Work Order is effective March 8, 2022, and is hereby made a part of and incorporated into the
Master Services Agreement No. 01-STE-0106 dated June 28, 2002 and any Addendums to Agreement No. 01-STE-
0106 (“Agreement”) by and between Computer Sciences Corporation, a DXC Technology company (“CSC”) and City
of Nashua (“Customer”), each a “Party”, together the “Parties”, located at 229 Main Street, P.O. Box 2019,
Nashua, NH 03060. In the event that any provision of this Work Order and any provision of the Agreement are
inconsistent or conflicting, the inconsistent or conflicting provision of this Work Order shall be and constitute an
amendment of the Agreement and shall control, but only to the extent that such provision is inconsistent or
conflicting with the Agreement.
CSC and Customer hereby agree as follows:

Upgrading Assure Claims to the latest version - $0
Assure Claims Upgrade Training
Assure Claims Configuration Training
Sisense Training
Database Analysis
Assumes no Customer Work to Analyze for Upgrade
Configuration Consulting
Data Analytics /MMSEA Upgrade Consulting
Upgrade Lawson Employee to Assure Claims Interface
Assumes Customer is using Microsoft SQL Database
Upgrade to latest version of Assure Claims
Project Management (44 hours)
Estimated total hours — 93@ $132.00 per hour (reflects discount from $165.00 per hour)
Estimated total training & Consulting days — 7 @ $1,760.00 per day (reflects discount from $2,200.00 per day)
Data Clean Up Services
e Data cleanup Efforts: 40 hours @ $125.00 per hour
e Project Management Efforts: 10 Hours @ $132.00 per hour
Risk to Lawson Indemnity /Risk to Lawson AP
e Utilize Sisense to complete Risk to Lawson Indemnity /Risk to Lawson AP as outlined in Customer
Attachment.
e 30hours; 8 hours of Project Management.
