1.05 SUBMITTALS: IN ACCORDANCE WITH REQUIREMENTS OF GENERAL
SPECIFICATIONS, SUBMIT THE FOLLOWING:
A. Electronic files of complete job mix formula shall be submitted to the Engineer
at least two weeks before any of the work of this section 1s to begin.
B. Delivery slips shall be furnished with each load of mix delivered to the project.
Information shall include:
Vehicle identification.
Date.
Project.
Identification of material.
Gross, tare and net weights.
Signed by the bituminous concrete producer.
Stamped by a licensed public weighmaster.

C. Material test results performed by a certified laboratory shall be submitted to the
Engineer for crushed gravel subbase proposed for the project. If required by the
Engineer, samples shall be submitted to the Engineer.
