We, undersigned, do hereby assert that this proposal is submitted without any collusion with any
other person or entity submitting a proposal pursuant to this RFP;
That this proposal is submitted without any conflict of interest to the best of our knowledge and
we intend to comply and conform to the no conflict provisions of the RFP.
That this proposal, as noted in the footnotes regarding references, is submitted without the named
references having been offered or provided with any benefits, products, discounts or other in-
kind services or products in exchange for providing a reference.
We have reviewed the 2™ addendum and have made a sample response identifying our
exceptions, objections and corrections with proposed terms to the sample/draft contract issued
via addendum 2/23/22 and incorporate them herein (see attached). We reserve the right to make
exceptions, objections and corrections to any final proposed contract once said contract/proposed
agreement is presented to us.
